By traveling around Argentina, from east to west and from north to south, in addition to performing activities in countries in the region such as Uruguay, Bolivia, Paraguay and Ecuador, we have gained experience working with entities that provide health care on the seashore, at the foot of the mountain range, and in other latitudes. One of these establishments, Sanatorio Argentino, provides services in the Province of San Juan a nd has a Teaching and Research Committee. This article describes the work done by Sanatorio Argentino on the relationship between LASA medications and patient safety vulnerability.
The detection of medication-related adverse events (AE) represents a crucial step in the development of prevention strategies and the improvement of the use of medications in every stage of the system. Several approaches have been implemented in Sanatorio, raising the personnel's awareness about voluntary reporting, which has led to a reduction in the number of errors made when dispensing and administering medications. This reassures patients and prevents the occurrence of AE that may worsen the condition or even cause death. Another great challenge faced by Sanatorio was the development of methods to detect flaws in the medication use system, even if the AE does not reach the patient. The examination of potential AEs helped them identify not only where the system failed (the error), but also where it worked (the interception).
